With our head office based in Bolton town centre, we employ over 400 staff in a variety of clinical and non-clinical roles, based in a variety of locations across Bolton and beyond.

Enhanced Access Service

Bolton GP Federation provides evening and weekend appointments on behalf of 6 PCNs across Bolton. Patients can book appointments in one of our hubs by contacting their own GP practice.

Appointments are available:
Monday to Friday – 6:30pm to 9:30pm

Saturday – 9am to 5pm

Primary Care Network Support

We currently support 6 PCNs in Bolton.

The networks provide the structure and funding for services to be developed locally, in response to the needs of the patients they serve.

Primary Care Networks are geographical groups of GP practices, community, mental health, social care, pharmacy, hospital and voluntary services working together to focus on local patient care.

They are small enough to provide the personal care valued by both patients and GPs, but large enough to have impact and economies of scale through better collaboration between practices and others in the local health and social care system.

Support to GP Practices

Supporting practices is a Bolton GP Federation priority.

Our role in supporting practices is developing and growing in response to what they need.

We currently support them by:

  • Delivering Extended Primary Care – by providing evening and weekend appointments on behalf of practices.
  • Facilitating Practice Manager peer support.
  • Providing Practice Manager capacity support/cover.
  • Supporting care homes through multi-disciplinary team meetings.
  • Promoting practice job vacancies.
  • Provide Ardens Manager, an evidence-based resource that promotes best practice, medicines management and patient safety.

Circulation Health

The Circulation Health project is a neighbourhood-based, multidisciplinary care model designed to proactively identify, engage, and manage patients with circulation health conditions, including chronic kidney disease, cardiovascular disease, particularly heart failure, and type two diabetes.

The service integrates primary care, local authority and the voluntary and community sector to deliver personalised, preventative and accessible care closer to home

Service aims

  • Improve early identification and diagnosis of circulation health conditions across local Integrated Neighbourhood Teams (INTs) and Primary Care Networks (PCNs).
  • Optimise treatment pathways in accordance with local and national guidelines.
  • Enhance patient experience and engagement through tailored interventions, education, and support.
  • Reduce health inequalities with targeted interventions.
  • Provide system efficiency, capacity and opportunities for financial sustainability
