Primary Care Network (PCN) Clinical Pharmacist

Bolton GP Federation is looking to recruit a Primary Care Network Clinical Pharmacist to support Bolton Central PCN.

You’ll work alongside GPs and the wider primary care team to provide expert medicines advice, support safe prescribing, undertake clinical medication reviews and help improve outcomes for patients, including those with long-term conditions and complex polypharmacy.

Hours: up to full time 37.5 hours

Salary: £45,316.71 to £51,858.02 a year

Primary Care Network Care Coordinator

Bolton GP Federation is looking to recruit a Care Coordinator on behalf of our Primary Care Networks (PCNs).

As Care Coordinator, you’ll work closely with GPs and primary care teams to support patients, particularly those with complex needs. They help coordinate care, connect patients with appropriate services, and ensure the right support is in place to improve patient outcomes and experience.

Hours: Full-time or part-time

Salary: £26,912.02 to £29,532.91 per year
